Request for Medical Records To obtain a copy of your medical record, please contact the WVU Healthcare Health Information Management Department via mail, MyWVUChart, or visit our office. We are located on the fourth floor of the Physician Office Center. Our office hours are 9 am to 4 pm Monday through Friday. There are two ways to obtain a copy of your medical records: 1) Complete the authorization form giving us permission to release your records, then mail or fax the form to: WVU Healthcare Release of Information PO Box 8049 Medical Center Drive Morgantown, WV 26506-8094 Fax: 304-598-4129 NOTE: We reserve our fax line for continuing care purposes due to high volumes. All other requesters please request via email preferably or mail. Click to download the Authorization for Release of Information form. 2) WVU Healthcare patients enrolled in MyWVUChart can request medical records using the electronic release request form within your MyWVUChart account. Records mailed to physicians for continued care or medical records less than 50 pages for personal use are free of charge. Login to MyWVUChart and click the Record Request button in the left navigation. Then click the WVUHealthcare Request form link and complete the form. To have the records mailed to you, list your name and address in the Doctor or Clinic fields. MyWVUChart is a free service. Learn more about MyWVUChart here. Please allow 7-10 business days for completion of a medical records request. Continuing care releases are given priority. Please mark the authorization if you have an upcoming appointment, and we will expedite your request. Records requested for personal use, attorneys, and insurance other than for disability or workers’ compensation will be billed prior to the release of the records. Medical records are housed for 20 years 2 months per hospital policy. Please note: For patients requesting a chart larger than 50 pages or for companies requesting healthcare records, charges apply and must be paid before records are released.
